Some queries and macros in Microsoft Access are not working or malfunctioning. Also the database plays up form time to time saying it is in an inconsistent state and needs repair itself. Furthermore, the generalation of address lists based on certain keywords does not work any more. - Due date: Flexible

I need to create a patient database on Microsoft Access. There will be one main table to keep all patient record. I would like a main page to display a dashboard (e.g. total number of patients in 'active' category, total number of patients under each doctor) as well as a link to another form to enter new patient records. I have a template of what I want these forms to look like, I need help with coding/linking the table to the form.
