Need help to install port for my TV internet and computer.

Require quote on network cable install. Layout image attach Blue X is where the NBN port is located, I require an RJ45 Data outlet installed with 2 ports. The 2 ports will need to run Cat6 cables to the second level and terminate to each bedroom marked with a Red X with R45 Data outlets in each.
