I'm an independent contractor (personal care worker). I only do two hours a week so I still receive Centrelink payment. I need to fill in a Mod F (Business Details) and it requires five additional documents: latest personal income tax return, a list of current balances for each bank account that the business operates, latest balance sheet, latest depreciation schedule and latest profit and loss statement (SU580 form from Centrelink).
