We’re looking for an experienced Jotform specialist to review, refine, and finalise our Safe Work Method Statement (SWMS) form. Previously, a Tasker created around 26 Jotforms. While most are usable, several key improvements are needed, especially for the SWMS form. General issues: • Time stamp records when form is completed, not when signed. (Sign-in/out times need correct tracking.) • Text sometimes doesn’t fit inside boxes — needs to auto-shrink or wrap. • Some forms don’t always email a saved link when partially completed — not critical, but would be good to fix. • GEO stamp currently saves coordinates — ideally show address on the form. SWMS form updates: • Column limit: Expandable columns (we can provide a layout). • Risk scoring: Currently tick boxes; should allow numerical entry (Likelihood × Consequence) or manual input. • Plant list: Should be a Yes/No dropdown, not free text. • Phone number: Accept standard Australian mobile format. • Sign-off column: Names repeat and layout broken — needs fixing. • Column overflow: Some columns spill to next page; layout adjustment required. Additional request: We’d like two versions: A blank version for custom input. A pre-populated version with common items filled, still allowing extra input. Attention to detail and strong Jotform experience required. Goal is a polished, fully usable set of forms for our team.
