The form needs to suit advanced skin, laser and beauty treatments and reflect a clean, luxe clinical aesthetic using neutral/beige and gold tones that match our branding. I would like a master consultation form created in Jotform that is professional, easy for clients to complete and fully editable so I can update, duplicate and adapt it later for treatment-specific consents.
