I have a form embedded in my Shopify website using Fillout. I would like to have the form recreated as a Google Form. You can view the form and amount of fields by googling Mandala Care Group and clicking on the menu ‘Farm Program Intake Form’. I then want it displayed as a clickable button in the top menu of the website, when they click the button the form comes up. I can then hide the current page.
