Seeking holistic health coach for both diet and PT

Hi there, thanks for reading my enquiry. Here’s my story:
I have struggled with health and fitness for years, trying to achieve specific goals but always ending up with undesirable drawbacks. I have tried many different diets and exercise regimes, and none of them really worked. I kept and stuck to the plans and worked hard but the results simply weren't happening.
I'm at a point in my life where enough is enough. I feel myself falling into a downward spiral with my energy, fitness and strength levels being significantly detrimented, through no significant unhealthy reasons so I’m pretty pissed off to say the least. I am thankful that I'm not too old at 30, nor am I overweight or have any serious health complications. But, I do have specific goals, and I am determined to achieve them. I have faith that I can find something that works for me.
I am seeking a holistic service whereby the one person can advise me in all aspects of my health and fitness needs. Namely, one person to help me with not only my dietary intake, but also with my training schedule, measurements and also my overall wellbeing such as energy and focus. I have tried having separate people for these things in the past and it doesn’t work for me so I would like to build a professional relationship and pathway with a single practitioner in a holistic way.
My goals are fairly common and generic. I’d like to reduce my body fat percentage (currently around 20%), increase lean muscle mass/size/strength, increase fitness, reduce fatigue, increase mental focus, eat cleaner/healthier and most importantly become educated about how and what to eat and exercise that works for my body. This is all aiming to just improve my performance as a regular "personal athlete": I'd like to keep up with my peers of the same body size when it comes to cycling speed, weight lifting, running etc., because I just feel a bit "physically pathetic" but even though I do acknowledge that I am a worthwhile person in this world say academically, I would just like to be a better physical performer.
Being pragmatic, I recognise that these services don’t come cheap so I am looking for a service which is sustainable financially. I have offered an amount of $100 which I'm hoping will cover an initial 1hr consultation, or at least most of it. My budget is for approx. $100 per hour, negotiable. I would prefer to pay ‘per hour’ as I find that the common ‘package deals’ are very expensive over a short period of time (e.g. $1200 over 3 months) whereas the hourly rate would suit me much better (e.g. @$100 per hour, one hour a month, the program would cost the same over an entire year); I am looking at something long-term.
Also, it would be preferable if the trainer’s fees can be subsidised by my private health fund (nib) which I pay good money for, to make the service more affordable (and sustainable) for myself. My health fund rebates accredited dieticians but not nutritionists (however, I can always switch to another fund that rebates nutrition should the service fit). For the same reasons, it would also be ideal if the trainer is accredited so that I can claim back on that too, though this is less important because my fund pays back double the amount with dietetics versus PT and I always use up the PT portion of my fund with gym fees. Whilst being able to claim back on my health fund by being accredited is not essential, I am more likely to select a professional that is accredited/qualified in both PT and dietetics/nutrition simultaneously.
By seeking accredited professionals, I am trying to avoid advice that is 'home-brew'; rather ideally professional and research backed. I’m certainly not looking for ‘e-books’ and those ‘quick-fix’ online/DVD programs; I need a health and life coach that will walk alongside me in this journey with the occasional (~ once per month) email should I have a brief inquiry.
The dietetics should support my aim to eliminate unnatural and introduce organic, raw and wholesome foods into my diet whilst also accommodating my lifestyle and personal limitations. I have done it before so I know I can do it again. My trainer should be able to work with my unique circumstance to produce a dietary plan that works for me that I can sustain.
The fitness aspect is probably much simpler: really I'm just looking for someone to give me clear guidance as to what exercises to do, how and when, keeping in mind that I have already had extensive experience doing many types so I don't need face-to-face PT as such, more just some guidelines and a 'program' to follow.
I am fairly independent and so I do not want to see my trainer on a weekly or fortnightly basis; I can get myself to the gym/running track etc. and do the prescribed exercises myself. And, with a clear and simple guide, I can get my stuff from the store and cook myself also. Instead, I'm looking for my trainer to meet up with me on a 4-6 week basis (perhaps more frequently in the beginning stages) that will keep track of my metrics: weight, muscle size, body fat percentage, strength and fitness tests, VO2, blood pressure etc.
In the long-term, I’m hoping that my trainer will thoroughly educate me as to how *my* body responds to foods and exercises, so that I can then carry a healthy lifestyle sustainably well into the future. I really need a guide that can help me understand what to eat, and how to train to get the body, strength, fitness and energy that I believe I deserve.
