Personal trainer, 2 days a week for two people. Ideally someone who also specializes in knee rehabilitation. Few questions on my knee issues so in summary: • Main issue (knee): Patellofemoral pain (pain around/behind the kneecap) driven by a focal full‑thickness cartilage injury on the left trochlea (the groove the kneecap tracks in). • Secondary knee findings: “Hoffa’s fat pad” irritation/impingement signs (both knees, more on right), mild cartilage wear on the right trochlea, and mild proximal patellar tendinosis on the right. • History of sciatica/low back pain; hard to fix this due to knee challenges (squats etc) Symptoms / triggers • Pain is worst with loaded knee bend: deep squats (especially with weight) and stairs. • Leg extensions: extension is OK, but pain increases on the return/bending phase (eccentric control).
